I beat Quarantine Zone, the game played the cut-scene at the end of Quarantine Zone, began loading for the next level, and, after finishing loading, I got blown back to the main menu saying "Insert the Original Halo 2 Disk". I click resume game and the last save appeared to be on Sacred Icon. So I exit to the main menu and go to select level and select Gravemind. It loads and when it finishes loading, I get blown back to the menu with the same error screen.

In your Halo 2\maps directory, make a copy of "07a_highcharity.map" and name it "07a_highcarity.map". Don't rename or replace the existing file, you'll need BOTH files present, one with the typo and one without. Then just fire up Halo 2, select Campaign/Select Level/Gravemind (or Resume if you've got the game saved just before the Gravemind level) and enjoy!

Let me start by saying "Thank you" to Linken as I was unable to play Gravemind until I saw his solution.
I found that even after using Linken's solution, once I re-booted or turned off my PC and then tried to load Halo 2 and resume my latest Gravemind checkpoint, I would get the 'Insert original Halo 2 CD" error message again.
My solution to this was to quit Halo 2, delete the "07a_highcarity.map" map that I created before and redo Linken's solution:

Then I loaded Halo 2, selected Campaign and then selected Resume and voila! my latest Gravemind checkpoint was alive and well.
I hope this works for others as it was a pain in the butt having to finish this level in one sitting before I tried this.

are you sure that you have
are you sure that you have the game completely installed? did you tamper/mess with any of the game files/folders? Did you have enough uncompressed hard drive space while installing/playing the game? Did you mod Halo 2 vista in any way, shape, or form? Did you have at least 2 Gigabytes of free space after installing the game?
I recommend backing up your save game/player profiles to a seperate folder, then re-installing Halo 2 Vista, then you can open the folder replace your player profiles/save games. if that doesnt work then i dont know.
